Your PCP checks TSH. We check TSH, free T3, free T4, reverse T3, and thyroid antibodies (TPO and TgAb). DUTCH stands for Dried Urine Test for Comprehensive Hormones. Instead of a single blood draw that captures one moment, DUTCH collects samples at multiple points over 24 hours. That means we can map your full cortisol curve (not just a morning snapshot), see how your body metabolizes estrogen through 2-OH, 4-OH, and 16-OH pathways, measure androgen conversion including DHT, and check melatonin production. Blood tests cannot show any of this. DUTCH is the single most informative hormone test available.
Two reasons. First, standard reference ranges are built from everyone who walks into that lab, including people with undiagnosed conditions and people on medications. Allergy testing measures IgE responses, the ones that cause immediate reactions like hives or throat swelling. Food sensitivity testing measures IgG and other delayed immune responses that take 24 to 72 hours to show up. That delay is why you cannot figure them out on your own. You feel bloated on Thursday and have no idea it was the eggs on Tuesday. Common sensitivity symptoms include fatigue after meals, brain fog, joint pain, skin flare-ups, and chronic bloating.
